TRANSFER CREDENTIALS (Formerly referred to as "Honorable Dismissal")
Guidelines
- A student in good standing who desires to sever his/her connection with the university must present a written petition to this effect, addressed to the College Dean. If the petition is granted, the student shall be given transfer credential.
- A transfer credential is issued only upon approval of the written petition, and after the student has cleared himself/herself from all accountabilities in the university.
- Transfer Credential is issued only ONCE.
Procedure
- File a written petition/request to the College Dean stating your reason for transfer.
- Once approved, present it to the Office of the University Registrar/Campus Registrar together you're your accomplished clearance.
- Accomplish the Request form. and secure the required signatures.
- Submit the form to the Registrar staff for assessment of fees.
- Pay the assessed amount at the Cashier's Office.
- Go back to the Registrar's Office and show the Official Receipt.
- Claim the Certificate of Honorable Dismissal/Transfer Credential on the scheduled date of release.
