TRANSFER CREDENTIALS (Formerly referred to as "Honorable Dismissal")



Guidelines

  1. A student in good standing who desires to sever his/her connection with the university must present a written petition to this effect, addressed to the College Dean. If the petition is granted, the student shall be given transfer credential.
  2. A transfer credential is issued only upon approval of the written petition, and after the student has cleared himself/herself from all accountabilities in the university.
  3. Transfer Credential is issued only ONCE.

Procedure

  1. File a written petition/request to the College Dean stating your reason for transfer.
  2. Once approved, present it to the Office of the University Registrar/Campus Registrar together you're your accomplished clearance.
  3. Accomplish the Request form. and secure the required signatures.
  4. Submit the form to the Registrar staff for assessment of fees.
  5. Pay the assessed amount at the Cashier's Office.
  6. Go back to the Registrar's Office and show the Official Receipt.
  7. Claim the Certificate of Honorable Dismissal/Transfer Credential on the scheduled date of release.
